Chapter 1 - THE FIRST CUT

Lily Carter’s heels struck the hospital mattress again and again.

The sound was wrong for a pediatric trauma room.

Rubber against vinyl.

Metal rails rattling.

A five-year-old sobbing so hard she could barely breathe.

“Please cut it out! There is something moving inside and biting me!”

She wore two purple rain boots even though the afternoon outside was dry and warm.

Not one pair.

Two.

An oversized outer pair had been forced over a smaller pair beneath it, making her legs look strangely heavy from the knees down.

Her right arm was already splinted from an obvious injury.

A dried cut remained above her eyebrow.

Every time nurse Chloe Evans reached toward the boots, Lily jerked away as though touching them would trigger something worse.

Then Greg Carter lunged across the room.

“Leave her boots alone! We’re leaving now!”

He smelled of stale alcohol, sweat, and old cigarettes.

His shirt was wrinkled. His eyes were red. But nothing about his movement was slow.

He shoved past Chloe and reached for Lily.

I stepped between them.

“No.”

“She’s my daughter.”

“She is a patient.”

“I said we’re leaving.”

Behind me, Sarah Collins triggered the security alarm.

Two guards entered almost immediately.

Greg tried to push through me.

They caught his arms and forced him backward.

“You have no right!” he shouted. “She fell. I brought her here. I can take her home.”

Lily folded over her boots.

Her entire body shook.

I crouched beside the bed.

I had treated terrified children before.

Fear of needles.

Fear of surgery.

Fear of strangers.

This was different.

Lily was afraid of what would happen if we helped her.

I kept my voice quiet.

“Tell me what he made you hide.”

Her wet eyes moved toward Greg.

He stopped shouting for one second.

That second mattered.

Lily’s fingers slowly loosened around the top of the outer boot.

“He said if doctors saw it…” Her voice broke. “I’d never come home.”

Greg began struggling again.

“She’s confused!”

Sarah moved beside me with trauma shears.

I looked at Lily.

“Can Sarah cut the outside? We won’t pull anything until you say.”

Lily nodded once.

Sarah slid the blunt edge of the shears beneath the outer seam.

The first cut made Lily scream.

Not from the scissors.

From anticipation.

I put one hand near her knee without touching.

“You’re safe right now.”

Sarah cut again.

The rubber separated.

A faint, unfamiliar odor escaped.

Chloe’s expression changed.

Sarah stopped.

I leaned closer.

Something inside shifted.

Not enough for me to identify.

Enough to know Lily had not imagined movement.

Greg saw our faces.

“No!”

His voice cracked.

“Don’t touch it!”

Sarah opened the seam another inch.

Chloe stepped backward.

Her face had gone white.

I saw shapes where no shapes should have been.

Layers.

A hidden lining.

Something trapped between them.

And beneath all of it, Lily’s ankle was red and swollen.

I raised one hand.

“Sarah… don’t touch anything.”

Greg began shouting at the guards.

I never looked away from the boot.

“Call police.”

Sarah froze.

“Marcus—”

“Now.”

Chloe reached for the emergency phone.

Greg’s struggling became frantic.

“You don’t know what you’re looking at!”

Maybe he was right.

I did not know yet.

But a five-year-old had begged us to cut open boots her father was desperate to keep closed.

Whatever was inside had already become more than a medical problem.
